Billing & Insurance

CareCredit Learn MorePayment for non-insured services is expected at each visit. Our centers accept cash, personal checks, and major credit cards including MasterCard, Visa, and Discover. We also offer financing and payment plans for those requiring care to save a patient’s vision.

Insurance

We accept Medicare and Medicaid assignments and participate in Aetna Medical, BCBS, CHAMPUS, CIGNA Medical, OptiCare, TRICARE, United Health Medical, and numerous other insurance PPO programs. We also participate in the Medicare PFFS plans. As a service to you, we will be happy to process your insurance forms. Insurance policies are an agreement between the patient and their insurance company. It is the patient’s responsibility to pay any deductible amount, co-insurance, or other balance not paid by their insurance the day of their appointment.

Some insurance companies require patients to obtain a referral form from their primary care doctor to obtain medical attention. Patients who do not have prior authorization will be responsible for the entire cost of the visit.

Medicare & Medicaid

If you have Medicare and your address has changed, please contact the Social Security Administration Office by calling toll-free (800) 772-1213. An incorrect address may delay or deny your claim.

Medicaid patients who are between 21 years of age and Medicare age are responsible for a $3 co-payment which must be paid at the time of the visit.

Financial Hardship

If care is needed to maintain or save a patient’s vision, we will not deny services to anyone based on financial status. If you anticipate any difficulty in paying for your visit or surgery, please contact our financial counselor so that we can make care arrangements.
