We accept Medicare and Medicaid assignment and participate in Aetna, BCBS, CHAMPUS, CIGNA, OptiCare, TRICARE, United Health and numerous other insurance PPO programs. We also participate in the Medicare PFFS plans. As a service to you, we will be happy to process your insurance forms. Insurance policies are an agreement between the patient and their insurance company. It is the patient’s responsibility to pay any deductible amount, co-insurance or other balance not paid by their insurance the day of their appointment.
Some insurance companies, including Carolina ACCESS Medicaid, require patients to obtain a referral form from their primary care doctor in order to obtain medical attention. Patients who do not have the prior authorization will be responsible for the entire cost of the visit.
Medicare & Medicaid
If you have Medicare and your address has changed, please contact the Social Security Administration Office by calling toll-free (800) 772-1213. An incorrect address may delay or deny your claim.
Medicaid patients who are between 21 years of age and Medicare age are responsible for a $3 co-payment which must be paid at the time of the visit.
If care is needed to maintain or save a patient’s vision, we will not deny services to anyone based on financial status. If you anticipate any difficulty in paying for your visit or surgery, please contact our financial counselor so that we can make arrangements for care.
